Prior to your first day of classes at UM, please complete your required INCOMING STUDENT MEDICAL FORM.

学生健康服务 (SHS) is an on-campus clinic that provides direct, basic medical care to all undergraduate students. SHS also serves as a referral source for students needing care beyond that available on campus. Expenses incurred from care provided off campus are the responsibility of the student. 的se services are available to currently enrolled, degree-看到king students who also meet state legal and ethical limits of services as outlined by the Alabama Board of Medical Examiners. Services provided are covered by undergraduate tuition and fees. Graduate students and continuing students 看到king services during the summer may elect to pay the semester fee to access services. Student Eligibility Information

Required Health Information for New Students residing in U.S.
ALL REQUIRED HEALTH INFORMATION MUST NOW BE SUBMITTED ONLINE.
的 Incoming Student Medical Form must be completed online by the student and submitted along with proof of measles immunity uploaded and attached to the form. All required health information is due before the first day of class.
For most students, proof of measles immunity will be a copy of the immunization record indicating 两个 separate dates of MMR (measles, mumps, rubella) vaccination. This document can be obtained from the student’s previous school attended, the physician’s office which administered the vaccinations as a child, or the county health department in which the student grew up. If this record is unavailable, proof of measles immunity can be acquired through blood testing (麻疹效价) at a physician’s office. If not immune, the MMR vaccination series must be completed. Even though Varicella (Chicken Pox), 破伤风, Hepatitis B and Meningitis vaccines are not required, they are strongly recommended.
的 Incoming Student Medical Form includes a Tuberculosis Risk Questionnaire. A TB test is not required unless the student is determined to be at risk. If required, the student will be contacted by Health Services.

Required Health Information for International Students
